Documenting your inability to work

If you are recently disabled, consider getting medical attention to determine your condition. The Social Security Administration reviews applicants’ medical records looking for proof that their conditions prevent them from working. How do I get my medical records?

You need to provide proof of disability when applying for disability benefits. To get your medical records, the National Disability Benefits advises you to contact all your doctors to request your medical records. However, you may still proceed with the application even if you do not find all of your medical records.

Medical practitioners

Compile a list of medical practitioners related to your condition because the SSA will require a list of all medical providers that you saw. You will be required to provide the names, addresses, and phone numbers of all medical practitioners, emergency rooms, clinics, therapists, and medical facilities that you visited because of your condition. If possible, provide dates when you saw each practitioner, including dates of admission and discharge.

Provide all your worker’s compensation claims to the SSA as well as all information related to the claims including settlement agreements, dates of injury, claim numbers, proof of other disability payments that have been made.
